Operator’s Compliance Packages
IVES Operator’s Compliance Packages contain everything necessary to train, evaluate and document operators of forklifts, aerial boomlift, scissor lift and combined aerial lifts, loaders, excavators and mobile cranes. Each compliance package contains an equipment specific workbook, theory test, evaluation form, certificate of completion and folder with data sheet and operator wallet card – enough material to train and document that process for one operator. We recommend all IVES Certified Trainers use our Operator’s Compliance Packages to document the training and evaluation of each operator.

Operator’s Compliance Packages include everything a Certified Trainer needs to train and document an operator’s certification, including:
Equipment-Specific Operator Reference Manual: This is the primary training aid used when certifying equipment operators and is the main source of theory information conveyed to the trainee. Manuals are self-paced with equipment-specific technical information and quizzes designed in a “test as you go” format that confirms learning and enhances trainee retention.
Operator Theory Test: A written test consisting of multiple-choice and essay type questions intended to test the knowledge of the operator on the specific type of equipment they are being trained to operate.
Operator Practical Evaluation Form: For the program trainer’s use, the evaluation form records the score, grade and pertinent details of the operator’s practical driving test. An essential piece of documentation, it records the demonstration of competence delivered by the operator.
Certification Folder
: The Certification Folder should act as the official record of certification in case of a regulatory audit or legal proceedings. This file folder, along with the Record Sheet, is to store the original Operator Theory Test, Practical Evaluation Form and any other documentation related to the employee’s certification.
Record Sheet: An integral part of the Certification Folder, the Record Sheet records all information relevant to the operator’s certification. Information recorded on the Record Sheet will 
include the operator’s name, company information and certification number in addition to specific information about the trainer, equipment, application and, of course, training dates, locations and test scores.
Certificate of Completion: This 8½” x 11″ certificate displays all the essential information of the certified operator including name, date of certification, certification number issued, type of equipment authorized to operate (suitable for framing).
Operator Certificate (Wallet Card): A wallet-sized version of the Certificate of Completion for the operator to keep on his/her person.
